    Exploring individuals perception of non-sponsored branded user generated content, the affect on its use and the outcome of digital engagement on Instagram

    With the rise of social media importance came the rise of the user and its role in the market. Users become actors, advisors, critiques, advocates and they reveal their nonsponsored branded voice. This study proposes an examination of the User Generated Content (UGC) and its influence on the social media platform Instagram. Regarding the existent literature, this research suggests a connection between three dimensions, value, use and digital engagement on social media platform, Instagram. More specifically, the proposed model established that depending on the value each individual attributes to non-sponsored branded UGC, it will lead to different types of usages. These types of use will then lead to two possible digital outcomes on Instagram. Using the data gathered from the 402 responses, the model was tested using PLS-SEM and conclusions were drawn. All the hypothesis tested were validated and results confirmed that there is a positive connection between all thee dimensions, UGC value, UGC use and digital engagement. Consequently, results also indicated that the most significant relationship is between the social value of non-sponsored branded UGC and UGC contribution, followed by the functional value of non-sponsored branded UGC and UGC consumption. In the end of the research, the meaning of the findings and the future possible research were deliberated.Com a ascensão da importância das redes sociais veio o crescimento do utilizador e do seu papel no mercado. Os utilizadores tornam-se atores, conselheiros, críticos, defensores e revelam a sua verdadeira voz. Este estudo propõe examinar o papel do user generated content (UGC) e a sua influência na rede social, Instagram. Com base na literatura existente, esta pesquisa propõem uma conexão entre três dimensões, o valor, o uso o do user generated content e o seu compromisso digital na rede social, Instagram. Mais especificamente, o modelo proposto estabelece que dependendo do valor que cada indivíduo atribui ao conteúdo não patrocinado gerado pelos utilizadores (UGC) levará a diferentes tipos de utilização. Estas formas de utilização de conteúdo poderão levar a dois possíveis efeitos de compromisso digital no Instagram. Usando os dados das 402 respostas obtidas, o modelo foi testado usando PLS-SEM e conclusões foram retiradas. Todas as hipóteses testadas foram validadas e os resultados confirmam a existência de uma conexão positiva entre as três dimensões, valor, uso e compromisso digital. Consequentemente os resultados revelam, também, que as relações mais significativas ocorrem entre o valor social do UGC e a contribuição para esse conteúdo, seguido pelo o valor funcional do UGC e o subsequente consumo desse conteúdo. No final da pesquisa, é proposta uma justificação para os resultados alcançados e são deliberadas as possíveis futuras investigações neste assunto

    Impact of blinded retrospective continuous glucose monitoring on clinical decision making and glycemic control in persons with type 2 diabetes on insulin therapy

The authors wish to thank the commitment of participants and care team at APDP e Diabetes Portugal.Background and aims: Blinded retrospective continuous glucose monitoring (rCGM) provides detailed information about real-life glycaemic profile. In persons with type 2 diabetes without adequate glycaemic control, the structured introduction of rCGM may be beneficial to sustain improvements in diabetes management. Methods and results: 102 individuals with insulin-treated type 2 diabetes, age less than 66 years old and HbA1c >7.5%, were recruited. Participants performed a 7-day blinded rCGM (iPro2) every four months for one year. Biochemical, anthropometric, and rCGM data was collected. Participants' and healthcare professionals’ perceptions were assessed. 90 participants completed the protocol. HbA1c was 9.1 ± 0.1% one year prior to enrolment and 9.4 ± 0.1% at enrolment (p < 0.01). With the rCGM-based intervention, a decrease in HbA1c was achieved at 4 months (8.4 ± 0.1%, p < 0.0001), and 12 months (8.1 ± 0.1%, p < 0.0001). A significant increase in time-in-range was observed (50.8 ± 2.4 at baseline vs 61.5 ± 2.2% at 12 months, for 70–180 mg/dL, p < 0.001), with no difference in exposure time to hypoglycaemia. After 12 months, there was an increase in self-reported diabetes treatment satisfaction (p < 0.05). Conclusion: In persons with type 2 diabetes and poor metabolic control, specific data from blinded rCGM informed therapeutic changes and referral to targeted education consultations on nutrition and insulin administration technique. Therapeutic changes were made more frequently and targeted to changes in medication dose, timing, and/or type, as well as to lifestyle. Together, these brought significant improvements in clinical outcomes, effective shared decision-making, and satisfaction with treatment. Registration number: NCT04141111.publishersversionpublishe

    Cervical cancer screening in low- and middle-income countries: A systematic review of economic evaluation studies

    Economic assessments are relevant to support the decision to incorporate more cost-effective strategies to reduce Cervical Cancer (CC) mortality. This systematic review analyzes the economic evaluation studies of CC prevention strategies (HPV DNA-based tests and conventional cytology) in low- and middle-income countries. Medline, EMBASE, CRD, and LILACS were searched for economic evaluation studies that reported cost and effectiveness measures of HPV DNA-based tests for CC screening and conventional cytology in women, without age, language, or publication date restrictions. Selection and data extraction were carried out independently. For comparability of results, cost-effectiveness measures were converted to international dollars (2019). Report quality was assessed using the CHEERS checklist. The Dominance Matrix Ranking (DRM) was used to analyze and interpret the results. The review included 15 studies from 12 countries, with cost-effectiveness analyzes from the health system's perspective and a 3% discount rate. The strategies varied in age and frequency of screening. Most studies used the Markov analytical model, and the cost-benefit threshold was based on the per capita GDP of each country. The sensitivity analysis performed in most studies was deterministic. The completeness of the report was considered sufficient in most of the items evaluated by CHEERS. The Dominance Interpretation (DRM) varied; in 6 studies, the HPV test was dominant, 5 studies showed a weak dominance evaluating greater effectiveness of the HPV test at a higher cost, yet in 2 studies conventional cytology was dominant. Publisher Copyright: © 2017 The Author.Aims Myocardial infarction (MI) is the leading cause of morbidity and mortality worldwide and results from an obstruction in the blood supply to a region of the heart. In an attempt to replenish oxygen and nutrients to the deprived area, affected cells release signals to promote the development of new vessels and confer protection against MI. However, the mechanisms underlying the growth of new vessels in an ischaemic scenario remain poorly understood. Here, we show that cardiomyocytes subjected to ischaemia release exosomes that elicit an angiogenic response of endothelial cells (ECs). Methods and results Exosomes secreted by H9c2 myocardial cells and primary cardiomyocytes, cultured either in control or ischaemic conditions were isolated and added to ECs. We show that ischaemic exosomes, in comparison with control exosomes, confer protection against oxidative-induced lesion, promote proliferation, and sprouting of ECs, stimulate the formation of capillary-like structures and strengthen adhesion complexes and barrier properties. Moreover, ischaemic exosomes display higher levels of metalloproteases (MMP) and promote the secretion of MMP by ECs. We demonstrate that miR-222 and miR-143, the relatively most abundant miRs in ischaemic exosomes, partially recapitulate the angiogenic effect of exosomes. Additionally, we show that ischaemic exosomes stimulate the formation of new functional vessels in vivo using in ovo and Matrigel plug assays. Finally, we demonstrate that intramyocardial delivery of ischaemic exosomes improves neovascularization following MI. Conclusions This study establishes that exosomes secreted by cardiomyocytes under ischaemic conditions promote heart angiogenesis, which may pave the way towards the development of add-on therapies to enhance myocardial blood supply.publishersversionpublishe

    Este artigo deriva de um conjunto de ideias experimentadas por discentes do Programa de Pós-graduação em Medicina e Ciências da Saúde da Pontifícia Universidade Católica do Rio Grande do Sul (PUCRS), em uma disciplina que procurou conhecer melhor o docente típico e suas atipias. O texto pretende auxiliar no entendimento de quem é o professor na área da saúde. Discute o sujeito e sua obra, para além do já conhecido modelo centrado na profissão da saúde que exerce e que procura ensinar. Antes de docente, o professor é uma pessoa. Como pessoa, precisa sentir-se estimada e estimar ao outro (aluno). Ao prosperar em seu trabalho, o docente tem compromissos para com o desenvolvimento intelectual e moral de seus alunos e o planejamento de ações para que exerça a percepção crítica da realidade. A relação ensino-aprendizagem com o educando deve favorecer a análise de valores necessários ao convívio social. Na área da saúde, é requisitado ao aluno aprender a fazer, sendo pouca importância referida ao aprender a conhecer, ao aprender a ser ou a viver em comunhão com os pares. É essencial ter a percepção de que o professor necessita, inicialmente, acolher a si próprio: entender quem é ele, como é e porque chegou ali. A construção da identidade profissional docente é um processo contínuo estabelecido pelo domínio de sua área de ensino, pelos conhecimentos pedagógicos voltados à aprendizagem dos alunos e experiência docente, além das relações sociais do cotidiano

    A patogenicidade do echovirus tipo 9 para camundongos recém-nascidos foi estudada, utilizando-se 12 amostras isoladas em cultura de células primárias de rim de macaco, a partir do liquor de crianças com meningite. Os animais inoculados com o fluido da primeira passagem em células desenvolveram paralisia flácida, após um período de 5 dias, com a morte ate o 8º dia. Os especimens originais de liquor não continham suficiente vírus para provocar sinais clínicos nos animais inoculados no período de 21 dias de observação. Ao exame histopatológico os animais doentes apresentaram miopatia necrotizante da musculatura paravertebral, língua e diafragma. Animais inoculados que não desenvolveram paralisia durante o período de observação apresentaram miosite discreta, sem que tenha sido encontrada necrose das fibras musculares.<br>The pathogenicity for baby-mice of 12 strains of echovirus type 9, isolated in primary monkey kidney cells, from liquor specimens of children with meningitis, have been studied. The animals inoculated with the first passage in tissue culture show after a period of 5 days, a flaccid paralysis and died by the 8th day after inoculation. The original liquor specimens did not contain enough virus to cause clinical signs in the inoculated animals, during the 21 days of observation. Histopathologicals studies in the sick animals, show a necrosis of the paravertebral muscles, tongue and diafragm. Inoculated animals wich did not develop paralysis during the observation period, show light miositis without necrosis of the muscle tissue
